257000 Lea Symbols 15 Line Pediatric Eye Chart
Pediatric distance visual acuity eye chart featuring LEA SYMBOLS for screening or testing individuals beginning at age 2 1/2.
The front side of the eye chart features pediatric symbols in proportionally spaced lines (ETDRS format) for monocular (testing one eye with the other eye occluded) and binocular (testing both eyes simultaneously) visual acuity testing or screening.
The opposite side features pediatric symbols
in the Massachusetts Visual Acuity Format for both left eye and right eye testing.
- Has a hole for hanging the chart on the wall. Includes response key, flashcards, and instructions. 17" x 21" (43 cm x 53.4 cm).

Since 2001, the West Virginia Vision Initiative for Children (VIC), an outreach program of the West Virginia University Eye Institute, has trained, equipped, and supported individuals who are now able to screen preschoolers for vision problems. VIC is now making this easy kit available to others involved with screening the vision of preschoolers.
Handheld flip chart featuring LEA SYMBOLS and HOTV optotypes to screen 3 and 4+-year-olds. The background is gray with the exception of a white rectangle spaced at 50% surrounding each line of symbols. Contains one practice line of 20/80 and critical line sizes of 20/40 and 20/32. Pages offset for easy flipping. 6 pages, 3.25" x 9" (8.25 cm x 22.9 cm).
